Social Security Card Request Form

What you will need to do is fill out the form below and hit the print button on your browser. Then make a copy of your driver license or your state issued identification card. If you need to replace a lost Social Security card, change the name shown on your card, or request a replacement card, you need to also complete Form SS-5, which is available for download here. You also need to complete the release letter, which you can download here. We are unable to retrieve new social security cards if you are age 18 or older and have never been assigned a number before, you must apply in person.

If you are living outside the United States, there is a different form, the Application for a Social Security Card (SS-5-FS) - Foreign Service that you can download here. Then send all necessary forms and proof of identification to the address listed below.

I understand that the Social Security office or above agencies can delay or deny social security cards at their own disgression, You also may on rare occasions be asked for addional information to process request.
