About Our Service
We are a private company that assists government documents such as Social Security Cards, International Drivers Permits, Passports, Visa's and Birth Certificates. We try to make retrieving documents quick & easy. Although getting a social security card is free we do charge a nominal fee to check your request & then hand walk your request thru the social security office. It is then processed immediately & mailed to you at the address that you provide.

Our fee is $45 per social security request.
How to Apply for a Social Security Card

Note: The required forms you will need are available in Adobe PDF format. Click here to learn all about what is required and to download this free software. There are strict guidelines, which must be followed, or your social security card will be delayed, or not processed at all.

If you need to replace a lost Social Security card, change the name shown on your card, or request a replacement card, you need to complete the Social Security order form, which is available for download here. You also need to print & fill out Form SS-5, which is available for download here. You also need to complete the release letter, which you can download here. We are unable to retrieve new social security cards if you are age 18 or older and have never been assigned a number before, you must apply in person. In addition to completing the form you need to furnish identification:

To get a replacement card, you need one (1) identifying document. Your replacement card will have the same number as your old card. Some documents that we can accept as proof of identify are:

  • Marriage or Divorce Record
  • Military Records
  • Employer ID Card
  • Adoption Record
  • Insurance Policy
  • Passport
  • Health Insurance Card (not a Medicare card)
  • School ID Card
For a name change on your card, you need documentation that shows your old name and your new name. Your new card will show your new name but will have the same number as your old card.
For example, an official marriage certificate or court documents showing a name change are required to change your name on your social security card.

Note: We cannot accept photocopies of documents. You must submit original documents or copies certified by the custodian of the record. Notarized copies are not acceptable. All documents submitted will be returned after the Social Security office inspects the documents.

If you were born outside the United States, you also must show proof of U.S. citizenship or lawful alien status.

Applying for Social Security Card is QUICK and it is EASY
Note: If you DO NOT follow these instructions below and give us the evidence that we need, we CANNOT process your application!!!

STEP 1 Fill out the order form, print & sign the form, and then print out the SS-5 & the release letter then print legible and sign the applications SS-5 & the release letter with BLUE or BLACK ink. Do NOT use pencil!! Follow instructions below.

How To Complete The Application

Most items on the form are self-explanatory. Those that need explanation are discussed below. The numbers match the numbered items on the form. If you are completing this form for someone else, please complete the items as they apply to that person:
2. Show an address where you can receive the card 7-15 days from now.
3. If you check "other" for CITIZENSHIP, provide an original document from the Federal/State or local agency explaining why you need a Social Security number and that you meet all the requirements for the benefit or service except for a number.
5. You do not have to complete this item about race/ethnic background. We use this information for statistical reports on how Social Security programs affect people. We do not reveal the identities of individuals.
6. Show the month, day, and full (4-digit) year of birth, for example, "1998" for year of birth.
8. You must enter the mother's Social Security number in item 8B. If you are applying for a number for a child under age 18.You must provide ID for both parent and minor if ordering a card for a minor.
9. You must enter the father's Social Security number in item 9B. If you are applying for a number for a child under age 18.
13. If the date of birth you show in item 6 is different from the date of birth you used on a prior application for a Social Security number card, show the date of birth you used on the prior application and submit evidence of age to support the date of birth in item 6.
16. You must sign the application if you are age 18 or older and are physically and mentally capable. If you are under age 18, you may also sign the application if you are physically and mentally capable. If you cannot sign your name, you should sign with an "X" mark and have two (2) people sign as witnesses in the space beside the mark. If you are physically or mentally incapable, generally a parent, close relative, or legal guardian may sign the application. Call us if you need clarification about who can sign.

How to Obtain the Form

How to Complete the Form
Complete the SS-5 using the instruction below. See pages 2 and 3 of the form for the evidence that you will need to submit with your application.
Where to Send the Form
Finally mail the SS-5 & the release, & order form to our office. Be sure to mail the originals or certified copies of your supporting documents along with the form. We will return your original documents right away. Once we have everything we need, the social security office will send your Social Security Card in about (7-10) days. We would suggest using an overnight delivery service to get your order to our office. Any orders that come in through an overnight delivery service are processed the same day.

How To Submit the Application
Mail the forms and your evidence documents to our office. We will return all your documents to you.

If you are age 18 or older and have never been assigned a number before, you must apply in person.

Your documents MUST be originals.

Evidence That Social Security Office Will Need

Caution: We cannot accept photocopies of documents. You must submit original documents or copies certified by the custodian of the record. Notarized copies are not acceptable. If your documents do not meet this requirement, we cannot process you application. We will return your documents.

If you need an ORIGINAL CARD (you have NEVER been assigned a Social Security number before), WE will not be able to assist you. You will need to contact the Social Security office for help.

When you go to their office you will be required to show.
  • AGE
  • IDENTITY
  • U.S. CITIZENSHIP (Birth Certificate)
  • LAWFUL ALIEN STATUS (Naturalization Certificate)


If you need a DUPLICATE CARD (no name change), you must show us proof of IDENTITY.

Important: If you were born outside the U.S., you must also show us proof of U.S. CITIZENSHIP (Record of Birth Abroad) or LAWFUL ALIEN STATUS (Naturalization Certificate).


If you need a CORRECTED CARD because of a name change, you must show us proof of IDENTITY.

To CHANGE YOUR NAME on our records, we need one or more documents identifying you by your OLD NAME on our records and you NEW NAME.

Important: If you were born outside the U.S., you must also show us proof of U.S. CITIZENSHIP (Record of Birth Abroad) or LAWFUL ALIEN STATUS (Naturalization Certificate).


Age: If you are requesting a replacement card for a minor, include a shot record or a report card. Photocopies are not permitted.
Identity: We can generally accept a current document that has enough information to identify you (e.g., signature, name, age, date of birth, and parents' names). WE CANNOT ACCEPT a BIRTH CERTIFICATE, HOSPITAL BIRTH RECORD, SSN CARD STUB, OR SSA RECORD FOR PROOF OF IDENTITY. Some documents that we can accept are:

- Driver's License   - Marriage or Divorce Record   - Military Records
- Employer ID Card   - Adoption Record   - Insurance Policy
- Passport   - Health Insurance Card (not a Medicare card)   - School ID Card

Important: If you are applying for a card on behalf of someone else, we must see proof of identity for both you and the person to who the card will be issued.


NAME CHANGE: If your name is now different from the name shown on your card, we need an identity document that identifies you by BOTH your old name AND your new name. Examples include a marriage certificate, divorce decree, or a court order that changes your name. Or we can accept two (2) identity documents. - One (1) in your old name and one (1) in your new name. (See IDENTITITY for examples of identity documents.

U.S. CITIZENSHIP: We can accept most documents that show you were born in the U.S. If you are a U.S. citizen born outside the U.S., show us a U.S. consular report of birth, a U.S. passport, a Certificate of Citizenship, or a Certificate of Naturalization.

ALIEN STATUS: We need to see a current document issued to you by the U.S. Immigration and Naturalization Service (INS), such as Form I-551, I-94, I-688B, or I-766. We CANNOT accept a receipt showing you applied for the document. If you are not authorized to work in the U.S., we can issue you a Social Security card if you are lawfully here and need the number for a valid nonwork reason. Your card will be marked to show you cannot work, and, if you do, we will notify INS.

ADOBE .PDF Documents It is now possible to print social security application forms in Portable Document Format (PDF). Adobe PDF files require Acrobat Reader software from Adobe Systems Inc. The forms you print must be clear and sharp. Applications that are blurred or illegible will not be accepted. They must be printed in black print on white paper. The paper must be 8 1/2 inches by 1l inches, with no holes or perforations, at least medium (20 lb.) weight, and with a matte surface. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable

Birth Certificates
We also can obtain a Certified Birth Certificate for you, in most cases, in 2-3 days. The certified birth certificates that we obtain for you can be used to travel to Mexico and Canada or any other country that requires a birth certificate and drivers license to enter, obtain drivers licenses or anything else that requires a birth certificate.
